Important Information about Medicare Open Enrollment
Medicare Open Enrollment runs October 15 through December 7 and this may be your only opportunity to change your plan for the coming year. It is strongly encouraged for people to review their current plan and learn what options are available so you can make the best decision for your individual situation.
The YSSC staff can help and can do interviews by phone or in person by appointment only. You must complete the form below and send it back to the Center. Please keep in mind that the only plans we can review are Drug plans and Advantage plans (HMO/PPO). Medigap Supplements DO NOT have open enrollment and supplement plan questions need to go to the company issuing your plan.

Remember, these plans change every year. Premiums and co-pays may go up, covered medications may change tiers or not be covered at all, and/or your doctor may no longer be contracted with your plan. This is why a review is very important.
If you would like to have your plan reviewed, you MUST fill out the form below COMPLETELY with your updated information and return it as soon as possible.
The deadline is December 7 to complete the reviews and make any changes.

- A complete list of all your medications. Medication name, strength, and how often you take it. (Prescription only) please do not include over-the-counter medications.
- A complete list of doctors and their specialty. (Unless you have a Medigap supplement, then this list is not necessary as you can go to any doctor)
- A list of your preferred local pharmacies. (e.g., Yellow Springs Pharmacy, Kroger, CVS)
